Reporting
When should you report?
All concerns that a vulnerable person has been harmed or placed at risk of harm as a result of abuse or neglect must be reported.
All concerns must be recorded in writing. Reports of concerns should be:
- Brief
- Factual (Who, what, when, where, how)
- Supported by available evidence e.g. summary of disclosure, body map etc
All concerns, incidents, allegations etc must be reported to the responsible manager/agency/ies at the earliest opportunity where possible in writing.
Who do you report to?
1. Emergency Services – where an immediate police or medical response is required e.g.:
- when immediate medical or police assistance is needed to help them, protect property or protect evidence,
- a serious crime is in progress, has just occurred or the person is at significant risk of harm.
- The safety of the vulnerable person and those of others in the environment is the priority
Call 999 for emergencies
Call 101 for all non emergency enquiries
2. Your line manager or other responsible manager, for example:
- The responsible home or acting manager where abuse/concern happens in a care home or nursing home
- The duty or registered manager where you work for a domiciliary care or nursing agency
- The ward manager or senior nurse if you work in a hospital or other health service
- The Unit/team manager or senior practitioner if you work for social services
3. The GP or other responsible medical officer – in all cases where injury has or may have occurred
4. The S&CS Social and Health Care Team or responsible care management team, where this is known.
